NO.4 Low back pain in primary care

For the management of acute nonspecific low back pain in primary care


 

This is the evidence-based guideline named European guidelines for low back pain. (Euro Spine J 2006 15: S131-S300) Let me introduce their recommendations.
   
  Advice to stay active: Advice to stay active and continue normal daily activities including work if possible.


  Prescribe medication, if necessary, for pain relief. Preferably to be taken at regular intervals. First choice paracetamol, second choice NSAIDs. Only consider adding a short course of muscle relaxants on its own or added to NSAIDs, if paracetamol or NSAIDs have failed to reduce pain.

 

 Consider (referral for) 
      spinal manipulation for patients who are failing to return to
      normal activities.


 Consider
      multidisciplinary treatment programmes in occupational

      settings for workers with sick leave for more than 4-8 weeks.


 Do not recommend
      back-school.
      behavioral therapy.
      massage.
      transcutaneous electrical nerve stimulation (TENS).


 Do not
       prescribe bed rest as a treatment.
       traction.
       specific exercises
        (for example strengthening, stretching, and extension exercises). 
       epidural steroid injections.


 Not included in this guidelines for acute nonspecific low back pain
       Acupuncture, Herbal medicine, Interferential therapy,

       Low-level heatwrap therapy, Neuroflexotherapy,
